Fundamentals of Hospice Palliative Care Program - Core Stream

Program Description:
Through discussion of dying and death and by becoming aware of the issues in each of the domains (disease management, physical, psychological, spiritual, end-of-life care/death management and loss and grief) that may impact persons and families living with life-threatening illness, formal caregivers are prepared and then challenged to apply strategies that can facilitate a change in the illness experience of those living with dying.
The Fundamentals Core is a 6-week, blended learning program that includes: independent reading, e-learning modules, coaching, in-person case-based learning sessions, peer-to-peer discussions and reflective activities.
Eligibility
In order to participate in the Core stream of the Fundamentals program, the learner must:
- Be a health care professional OR unregulated provider caring for people with a progressive life-limiting illness OR a student OR a volunteer connected with a service provider agency/hospice program intending to care for people with a progressive life-limiting illness
- Have a personal or organizational access to an Internet-enabled computer
- Have knowledge of base computer programs (or be willing to develop these skills)
- Be available to complete each Module of the program and associated learning tasks
- Have the ability to listen, learn from others, to question self and others, and identify new approaches
- Be willing to explore personal attitudes and beliefs
In order to participate in the Enhanced stream of the Fundamentals program, the learner must additionally:
- Be a Nurse Practitioner (NP), Registered Nurse (RN) or Registered Practical Nurse (RPN)
- Have successfully completed the Core stream of the Fundamentals program

Completion Requirements:
- Attendance at three, 3-hour case-based learning sessions
- Self-directed reading of the Program Guide
- Two peer-to-peer discussions
- 11 e-learning modules
- Reflective activities
- Course evaluation
Performance Objectives:
Following the completion of this course, the learner will:
- Demonstrate sensitivity, understanding and respect for the individuality of the person, family and team/caregivers
- Identify and understand issues associated with illness and bereavement (domains of issues)
- Effectively communicate with the person, family and team
- Recognize and report changes in any of the domains of issues associated with illness and bereavement
- Provide hospice palliative supportive care strategies
- Actively learn and develop personally and contribute as part of a team and organization
- Demonstrate sensitivity to the impact of personal attitudes, behaviours and values on the illness experience of the person, family and team/caregivers
